All dosimetry customers are provided with standard Dosimetry Reports, but there may be times when additional reporting is necessary. Our wide portfolio of ancillary reports helps your program remain compliant with regulatory requirements and promote a culture of safety.
Reports are accessed through myLDR, and applicable reports are deliverable to employees via Individual Dose Review (IDR). You can also download them for printing, emailing, or storage. Signature SuiteTM subscribers also have the additional benefits of digital signatures, participant delivery tracking, and inspection-ready records.
LANDAUER Ancillary Reports
Form 5: Provides the final dose received by a participant during the year or quarter as well as the lifetime dose. NRC equivalent, quarterly or annual options.
Form 5 - Termination: Details dose history for a participant for the last year as well as the lifetime dose. NRC equivalent.
Form 4: Provides the dose received by a participant during the year as well as lifetime dose broken out by employer. NRC equivalent, quarterly or annual options.
Fetal Monitoring Report: Displays fetal assigned dose for participants who have been pregnant any time within the past year.
ALARA Recap: Displays all participants in the subscribed account and indicates which participants exceed ALARA thresholds. Quarterly and annual options.
Radiation Dosimetry Report - Internal Dose: Includes organ dose, committed effective dose equivalent (CEDE) total, intake quantity by radionuclide, mode of intake and lung clearance class.
Multiple Employer Total Exposure Report (METERTM): Summarizes the doses received by participants at different facilities and aids in state and federal regulatory compliance.
ESTimate Report®: Displays estimated doses for participants who have not returned their dosimeters for a specific wear date.
Previous History Dose Report: Used for new customers who are shifting to LANDAUER products or for monitoring participants who were monitored by a previous employer.
Exposure Notification: Generated every time one or more dosimeters exceeds the dose value threshold that LANDAUER has saved for your account.
Report prices vary and can be quickly added to your account via the Subscription Center in myLDR.
